The Arizona Coalition for Change COVID Relief Fund was created to provide assistance and needs in the areas of: food, education, utilities, and rental assistance, for families of underserved/under-represented Black/African American, Hispanic/Latinx, & Indigenous/Native American communities in Maricopa or Pima County Arizona. Those who are financially affected by the impact of the COVID-19 pandemic, may fill and submit this form to apply for assistance.

NOTE: Although we would love to assist all who are in need of assistance, resources are limited. Due to our limit on funded resources, assistance may not exceed an amount of $800.00 per household. If your application is approved after the reviewal process, our COVID Relief Fund Outreach Team will contact you within 5-7 business days.
